Job Description:
As a Health Education Class Facilitator at Miramar Health Center
Kennewick, you will provide a critical service to your community by
recruiting patients to enroll in and also facilitating health and
wellness classes that will help them manage their chronic
conditions. Classes are delivered both online/virtually and in the
classroom. Classes include the Chronic Disease Self-Management
Program (CDSMP), Tomando Control de su Salud (Tomando), Diabetes
Self-Management Program (DSMP), and Manejo Personal de la Diabetes
(Manejo).
The schedule for this position is an average of 8 hours/week and
will require you to work some weekdays, evenings, and occasional
weekends. Some times are flexible.
What we offer
- $22.00/hour.
- Additional 4% pay for your bilingual skills.
Essential Functions/Responsibilities/Duties:
- Recruit and schedule patients to attend online and in-person
classes.
- Facilitate evidence-based curriculum for the programs listed
above.
- Actively engage participants during the classes, using your
knowledge of cultural differences to provide an environment where
people feel comfortable to share and ask questions. Continue to
engage participants by following up with them after class.
- Embrace and practice cultural sensitivity, equality, equity,
diversity, and inclusion in each part of the programs you are
involved with.
- Continue your own professional development by completing
trainings that are offered by YVFWC and other providers.
- Collect and compile participant data obtained from classes and
submit weekly reports to program leadership.
- Modify presentation delivery and content when program
requirements change.
- Consult with and provide class feedback from participants and
peers to program leadership.
- Refer patients to other YVFWC departments and programs that can
provide additional resources to help them manage their
conditions.
- Contribute to the accuracy of the department's billing for
classes.
- Use various computer applications and technology, including
Epic (electronic medical records), Microsoft Office, Zoom, and
Artera (patient communications).
Qualifications
- Education: High School Diploma or General Education Diploma
(GED).
- Experience: Six months experience as a volunteer, Promotor,
health educator, or a community resource information provider.
Computer skills in Word, Excel, PowerPoint, and Outlook. Basic
knowledge and experience with ZOOM video communications platform.
The ideal candidate has six months experience facilitating health
and wellness education classes, leading group discussions, and/or
has medical billing and terminology experience.
- Knowledge/Skills/Abilities: Bilingual skills are preferred.
Language needs are based on demographics of the area where you are
working. You must demonstrate the ability to communicate at level
10 on a language proficiency scale to receive bilingual
differential pay. Ability to successfully complete Tomando Control
de su Salud, CDSMP, Manejo Personal de la Diabetes, and/or DSMP
leader training and certification. Strong verbal and written
communication skills to effectively exchange, convey and receive
information. Public speaking skills to effectively present
information in a public setting. Take initiative to complete tasks.
Display a positive attitude in day-to-day situations and
interactions. Collaborate well with others. Demonstrate awareness
and sensitivity to different perspectives, and consistently respect
individual capabilities, culture, and personalities.
