Together, these specialists deliver a complete patient centered program of care for patients with cardiac conditions. The program delivers care in the outpatient setting throughout Central Oregon and inpatient hospital care at the St. Charles Bend campus.POSITION OVERVIEW: Under the clinical direction of the supervising provider provides quality patient and family centered care as a member of the health care team in accordance with St. Charles Health System mission, philosophy, policies, and procedures. Applies standards for professional nursing practice and focuses clinical care to enhance the experience for patients, families and all others. Responsible for daily patient flow. Use triage skills to make appropriate patient health assessments and anticipate physician's needs regarding patient care. This position interacts with the adult patient population.This position does not directly manage any other caregivers.ESSENTIAL FUNCTIONS AND DUTIES:Maintains and demonstrates an in-depth knowledge of nursing principles, practices, standards, and techniques, and applies this knowledge in accordance with St. Charles Health System policies and work instructions and within the laws governing nursing duties of an LPN in the State of Oregon.In coordination with supervising provider, assesses the patient's condition and needs using critical thinking and clinical judgment; implements appropriate nursing actions and collaborates with other disciplines to meet the patient's/family's physical, emotional, and other needs; and evaluates patient's progress. Anticipate physician's needs as they relate to the patients' medical care.Reviews incoming referrals, compiles necessary documentation and testing in conjunction with provider and works with the department to coordinate timely patient appointments and testing.Collects objective and subjective data from observations, examinations, and written records in an accurate and timely manner as appropriate to the patient's health care needs and context of care.Records medical history data, room's patients, take vitals, prepare patients as necessary for exams, performs EKGManages medication administration, actions, reactions and discharge teaching.Contacts patient and provides necessary education for upcoming testing or procedures.Prepares and follows-up with tests including labs, cardiac caths, MRIs and others.Performs telephone triage; communicates (both written and orally) effectively and maintains professional relationships with caregivers, medical staff, patients, and guestsSupports the vision, mission and values of the organization in all respects.Supports Value Improvement Practice (VIP- Lean) principles of continuous improvement with energy and enthusiasm, functioning as a champion of change.Provides and maintains a safe environment for caregivers, patients and guests.Conducts all activities with the highest standards of professionalism and confidentiality.
