Job Description Job Description Environment & Population Served Our facilities function as Skilled Nursing Facilities (SNFs) that offer: Long-term care for residents needing continuous skilled assistance and compassionate daily support. Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury. Collaborative care through a team-oriented approach involving nursing, therapy, and medical professionals working in unison. A varied patient demographic that includes those with orthopedic, neurological, cardiopulmonary, and post-acute care needs. The patient mix may differ slightly across facilities. As a therapy professional, your role will be to assist patients in achieving functional improvements and enhancing their quality of life within a nurturing, patient-focused setting. Position Overview The Speech Language Pathologist (SLP) delivers direct rehabilitation services in speech therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards. Qualifications and Licensure Requirements Graduation from an accredited speech therapy program (minimum MS or MA in SLP). ASHA certification (CCCs) OR eligibility for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification (CCCs) and/or meet ASHA credentialing standards. Once obtained, ASHA credentials must be maintained. Current registration/licensure in the relevant state, with an obligation to keep the license active and in good standing throughout employment. Preferred one (1) year of experience in post-acute care or a related environment. CPR certification required upon hire or to be obtained during orientation, with the necessity to keep it current throughout employment. Specific Job Responsibilities Engage in community health initiatives/projects as appropriate. Proficient in Microsoft Word, Excel, and email communication. Coordinate with patients, families, and support departments to effectively address patient needs. Exhibit proper body mechanics at all times. Demonstrate the ability to make independent decisions when necessary. Familiar with practices, procedures, and the laws, regulations, and guidelines governing operations in a post-acute care setting. Implement and interpret departmental programs, goals, objectives, policies, and procedures. Perform competently in all required areas, including patient rights, safety, and sanitation. Maintain professional relationships with colleagues, vendors, and others. Ensure confidentiality of all proprietary and sensitive information. Comply with organizational policies, including those related to harassment and compliance. Exhibit integrity and professionalism by adhering to the organization’s Code of Conduct and completing mandatory training. Core Functions Establish, evaluate, and adjust realistic, measurable, timely, and functional goals. (SLP CCC only) Responsible for supervising and assessing care provided by SLP CFYs. (SLP CFY only) Follow speech therapy treatment plans for patients under the guidance of the supervising SLP CCC, in accordance with Federal and State regulations. Document appropriately and in a timely manner. Utilize therapy software accurately and effectively. Provide excellent customer service and maintain a positive demeanor towards patients. Assist in the safe evacuation of patients when necessary. Demonstrate reliable and consistent attendance. Employ reasoning skills and sound judgment in decision-making. Communicate effectively and collaborate productively within an interdisciplinary team. Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently throughout the workday.
