opportunity for a Nurse Care Coordinator. The Nurse Care
Coordinator works as part of a Social Work and Nursing Care
Coordination team with the goal of achieving coordinated patient
flow through an inpatient stay, culminating in a safe and timely
discharge. Serving as an acute care case manager, this position
works closely with all members of the interdisciplinary health care
team, insuring that the team is aware of the patient's plan of care
and that all disciplines are working toward the same goals for the
patient.SCHEDULEFull-time days Monday -
Friday.RESPONSIBILITIESIdentification and monitoring of high
medical needs patients to ensure timely completion of consults,
procedures and tests, ongoing team communication, and facilitates
coordination between providers across the continuum of care.Lead
medical team and nursing huddles, and work creatively on removing
barriers to discharge due to medical complications and/or system or
team obstacles.Assist with ordering complex medical equipment and
in ensuring safe transitions of care.Develop and monitor outcome
measures related to care coordination, case management, and staff
and patient satisfaction with discharge planning, and providing
ongoing consultation and education to staff on how to achieve
improved patient flow through the UWMC
system.REQUIREMENTSBachelor's degree in Nursing.Master's degree in
Nursing or related field. Equivalent experience may substitute for
education requirement.Three years of professional nursing
experience.Able to communicate effectively in English, both
verbally and in writing.Effective interpersonal skills, leadership
skills, and clinical expertise.Ability to access information and
document in electronic medical record.LEGAL REQUIREMENTSCurrent
Washington State RN licensure at time of hire.DESIREDAdult
inpatient acute care nursing experience.Case management experience
preferred.UW Medicine includes Harborview Medical Center, UW
